A Support Coordinator provides assistance to a person with an existing NDIS plan to better understand, implement and manage their funds, while growing their ability to exercise choice and control over their supports.

Your Support Coordinator will:

• Clearly explain your NDIS Plan, discuss your goals and what funding you have to work towards these.

• Provide options for services and organisations so you can make the right choice for you.

• Link you in with services and check in regularly to see how these are going.

• Support you should you wish to change anything in your plan. 

• Prepare for your next NDIS plan.
